POP Integrated Wellness is a Detroit-based, veteran-led community health and human-services organization helping individuals move from identified need to completed connection. Led by DeAndre J. Martin, Founder & Lead CHW.
We support Medicaid and MI Bridges navigation, targeted outreach, compliant documentation, closed-loop referrals, and veteran-focused engagement — through flexible subcontract, embedded, and project-based models.
Our work is grounded in lived experience, systems knowledge, and relationship-based engagement. We serve individuals who are often overlooked by traditional models—not because services don't exist, but because navigating them alone is overwhelming.
Working knowledge of Epic documentation workflows, FindHelp-based resource referral, MI Bridges navigation, and Medicaid CHW requirements. Adult Mental Health First Aid trained.
